Emergency Medical Information Kit
When an emergency occurs, you want to be prepared with all of your medical information readily available. We have created an Emergency Medical Information Kit that you can download here, complete with your own medical and personal information, and store in an easily accessible location in your house.
This kit includes the following documents for you to fill out:

- Insurance Information
- Medication List
- Allergies
- Primary Care Physician Contact Information
- Advance Directive / Living Will*
- Health Care Power of Attorney Forms*
*Please note that the Advance Directive/Living Will and Health Care Power of Attorney forms must be witnessed and/or notarized in order to be legal and binding. You may also wish to consult your personal attorney for additional recommendations regarding these decisions.
We recommend clipping this entire packet onto your refrigerator so that you, a spouse, friend or even an EMS provider, can easily locate it in an emergency situation.
Diabetes Education Class
We offer quarterly Diabetes Education Seminars and monthly Diabetes Support Group sessions. A variety of topics are presented by a diabetes nurse educator, dietitians, pharmacists and physicians, including diabetes management, meal planning and carbohydrate counting, making smart choices when dining out, achieving better results with medication, and much more.
Community Health Seminars
West Chester Hospital has been steadfast in embracing the community and engaging in outreach events, including free monthly health seminars wherein leading physicians present information on topics such as healthy aging, diabetes, women’s health, and much more.

School Partnerships
UC Health’s tripartite mission includes a call to “educate and inspire the next generation of healthcare professionals.” With that goal in mind, we have
developed strong partnerships with numerous educational institutions in the community, including the Lakota Local School District, Butler Tech and its subsidiary Project Search, Hamilton City Schools, Xavier University, Sinclair Community College, and the University of Cincinnati. These collaborations have been strengthened through joint programs including career fairs for younger students and job shadowing initiatives for high school and college students, work-study programs, participation in Junior Achievement career days, and fun and educational programs such as our “Name the Robot” contest.

Speakers Bureau
With a network of hundreds of physician and clinical experts at our fingertips, we are pleased to help community groups find health education speakers and organize health screenings off-campus in the community. We also welcome nonprofit organizations to utilize our hospital conference rooms for events including meetings, seminars, parties and other health related needs.
