Our goal is to provide easy access for referring physicians and clinicans to consult with UC Health specialists and to coordinate new patient appointments to ensure the best possible care options for each patient's unique healthcare needs.

To refer a patient to one of our practices, please be prepared to provide the following information when you call for an appointment:

• Patient’s Information (name, gender, address, phone number, date of birth and insurance card)
• Reason for the referral, symptoms and diagnosis
• Referring Physician Information

It offers a single point of entry for specialty consultations as well as one-stop shopping for questions regarding UC Health services.
