Daniel Drake Center for Post-Acute Care specializes in helping patients who need more time to recover from catastrophic injury and illness. Often these are patients who have experienced traumatic injury, spinal cord injury or stroke, or are in need of advanced wound or pulmonary/ventilator care. Daniel Drake Center offers two levels of inpatient care – Long-term Acute Care (LTAC) and Skilled Nursing Care.
If you have a patient who could benefit from our long-term acute care services, we offer a streamlined referral process to facilitate a smooth patient transfer:
- To make a referral, call Daniel Drake Center at (513) 418-4365 or 1-800-948-0003 x 4365, or complete the form below and fax to (513) 418-2599.
- Within 24 hours, one of our assessment liaisons will provide a free patient evaluation to determine whether or not he/she meets the criteria for admission and also will speak with the family.
- If the patient meets the admission criteria, our assessment liaison will contact the patient’s insurance carrier to verify benefits, determine the need for additional assistance and obtain approval for a transfer to Daniel Drake Center.
- Once approved, our assessment liaisons will notify the referring facility’s discharge planner and work with our admitting office to coordinate the transfer.
