Our Skilled Nursing Care provides short-term skilled medical care and therapies to bridge the gap between hospital-level and a more independent level of care. It combines our expert physical rehabilitation and the high-quality medical/nursing services of our skilled nursing facility for specific patient populations (i.e., geriatric orthopedic rehabilitation).
Skilled Nursing Care clinical admission and continued stay criteria
- Medically Stable
- Age > 18
- Anticipated length of stay less than 30 days
- Rehabilitation potential with expectation for clinical/functional improvement, patient willing to participate in therapy program
- Pressure ulcer stage I-IV, wound care interventions < certified wound team, wound recs, mist therapy for non-healing wounds.
- Stable tracheostomy management.
- Respiratory treatments and interventions, nebulizer treatments, oxygen therapy adjustments, suctioning not to exceed Q8 hour interventions.
- Isolation, active infection with C Difficile, MRSA/VRE/MDRA
- IV Anti-infectives < 2 drugs/ < 4x/24 hours (maximum Q6 hrs)
- IV Fluids via PICC, peripheral line
- Nutritional management/tube feeding via G tube/PEG
- Onsite Physician 24hrs for urgent patient needs.
- Onsite labs, radiology and pharmacy
