Daniel Drake Center for Post-Acute Care specializes in helping patients who need more time to recover from catastrophic injury and illness. Often these are patients who have experienced traumatic injury, pulmonary/ventilator weaning, medically complex conditions, post-surgical complications, advanced wound care, spinal cord injury or stroke. Daniel Drake Center offers two levels of inpatient care – Long-term Acute Care (LTAC) and Skilled Nursing Care.
If you have a patient who could benefit from our long-term acute care or skilled nursing care, we offer a streamlined referral process to facilitate a smooth patient transfer:
- To make a referral, call Daniel Drake Center at 513.418.4365 or 1.800.948.0003 x 4365.
- Within 24 hours, one of our assessment nurses will provide a free patient evaluation to determine whether or not he/she meets the criteria for admission and also will speak with the family.
- If the patient meets the admission criteria, our assessment nurse will contact the patient’s insurance carrier to verify benefits, determine the need for additional assistance and obtain approval for a transfer to Daniel Drake Center.
- Once approved, our assessment nurse will notify you and work with our admitting office to coordinate the transfer.
A key to placement is making an early referral. In doing so, the likelihood of the patient meeting our criteria for LTAC is much higher. Unfortunately, patients who fail to meet our criteria often do so because too much time has passed since their injury.
