Six Month Program

This six month, predominately food-based plan is an option for individuals who want to obtain or maintain a healthy weight. Based on weight-management goals, this program incorporates some meal replacement products.

This highly personalized plan can start your weight loss journey or be the final step toward a healthier you. The Healthy Living program also can help weight loss surgery patients jumpstart their weight loss or get patients back on track if weight loss stalls. The rate of weight loss varies based on individual goals and needs.

Who is it For?

This program is for those who need to lose weight or want to maintain a healthy weight with the flexibility of a predominately food based meal plan. We recommend this program for individuals with a BMI of 27 or higher and ages between 18-64. Patients 65 and older require physician approval before participating.

What’s Involved?

Meal plans include a minimum of 1,200 calories per day during this six-month food-based program.  Some meal replacement products may be incorporated at the discretion of your physician or dietitian.

Monthly medical monitoring and group education classes are required. Routine office visits and blood work are prescribed by your care team to ensure successful weight loss.

What’s After?

For graduating patients, our office will work with you to transition your care back to your primary care provider.  We also offer a variety of support programs to help continue with your weight loss journey, including wellness and personal training programs through the Daniel Drake Center for Post-Acute Care, monthly support group meetings, an e-newsletter and more.
