The purpose of this research study is to follow and document the functional outcome and quality of life of patients who have had a stroke. Since stroke is the leading cause of disability, we believe this will assist in identifying and changing prevention

Criteria:

You Are Being Asked To Take Part In This Research Study Because You Are 18 Years Or Older And You Have Been Diagnosed With An Ischemic Stroke During 2015 And You Live Within The Counties Of Hamilton, Clermont, Boone, Kenton, And Campbell.
