Your First Visit at the Center for Reproductive Health

Upon making your first appointment with the Center for Reproductive Health, you will receive a welcome packet mailed to your home. This packet contains information confirming your appointment/consultation, map and directions to our location(s) and important medical forms that will need to be completed prior to your first appointment/consultation. On the day of your appointment, we ask that you arrive 10 minutes before your scheduled appointment time and complete and bring with you the following:

  • Insurance card and insurance coverage information
  • Drivers license/ photo ID
  • Completed forms
  • Insurance co-payment or self payment
    • We accept cash, personal check, Care Credit, Mastercard, Visa, AmEx and Discover.

Much of your first appointment/consultation is reviewing past medical records. It is important for us to have this information upon your first visit. We ask that you and/or your partner contact your referring physician or primary care physician to obtain medical records which can be sent to us before your appointment. They can be faxed to (513) 475-7601 (for the West Chester location) or (513) 585-3098 (for the Christ Hospital location). You may also bring your medical records/information to your first appointment. As you meet with one of our experienced physicians, you and your partner will have the opportunity to ask any questions that you may have and address any issues that might be of worry. Fertility concerns are best resolved in consultation with both you and your partner. It might be recommended that you and your partner have some additional medical testing to help identify the cause or source of fertility complications. Particular care will be taken so that only the appropriate tests relevant to you and your partner are conducted, saving both time and expense. If this is the case, we can schedule these during your first visit. After you have completed your initial consultation with one of our physicians, you will then meet with our dedicated financial counselor to discuss cost, insurance coverage and possible payment options/solutions.
